A leading pediatric health system is seeking an experienced pharmacy leader to guide an established Home Care Pharmacy through an important period of growth and organizational change.
This leader will oversee pharmacy operations, strengthen workflows, support Joint Commission readiness, and develop a team that plays an essential role in patient care beyond the hospital setting. The opportunity is to build on an established service while helping shape its future direction and expand its impact across the health system.
This position is well suited for a calm, strategic leader who understands home care or home infusion pharmacy and can bring trust, direction, and operational discipline to a highly regulated environment.
Lead Home Care Pharmacy operations and pharmacy personnel.
Guide the team through operational and organizational change.
Prepare the department for Joint Commission accreditation activity.
Lead USP <800> initiatives and strengthen compliance practices.
Assess workflows and identify operational and regulatory risks.
Improve pharmacy systems, processes, and services.
Ensure compliance with state, federal, health system, and professional standards.
Collaborate with multidisciplinary teams, vendors, and pharmacy leadership.
Develop employees and strengthen team performance.
Help advance the visibility and future growth of home infusion services.
Graduate of an accredited school or college of pharmacy, or completion of applicable foreign pharmacy equivalency requirements.
Current Minnesota Pharmacist license or ability to obtain licensure within three months of hire.
Current BLS certification or ability to obtain certification within three months of hire.
At least three years of pharmacist experience in a health system or related pharmacy setting.
Pharmacy leadership or management experience.
Home care or home infusion pharmacy experience strongly preferred.
Experience with Joint Commission standards or accreditation preparation strongly preferred.
Experience leading teams through significant operational change.
Strong employee development and communication skills.
Pharmacy project planning or implementation experience preferred.
PharmD, advanced business or administrative degree, or pharmacy leadership residency preferred.

The base salary range is $154,500 to $205,600 per year.
It’s an on-site role in Minneapolis, Minnesota.
Yes—either a current MN Pharmacist license or the ability to obtain it within three months of hire.
You’ll lead Home Care Pharmacy operations, manage staff, improve workflows, and support Joint Commission readiness.
It’s strongly preferred.
BLS is required or must be obtained within three months of hire.
At least three years in a health system or related pharmacy setting, plus leadership or management experience.
