• Deliver high-quality housing stability services, ensuring members receive exceptional care navigation.

• Conduct comprehensive assessments to identify members' housing needs and develop personalized care plans.

• Provide guidance and support to members navigating housing instability, addressing barriers, and facilitating access to resources.

• Collaborate with interdisciplinary teams to ensure coordinated care and support for members.

• Utilize expertise in housing instability to inform program development and service delivery.

• Maintain accurate documentation and data records in compliance with regulatory requirements and timelines.

• Other duties as assigned.

• Bachelor’s degree in social work, social services or related field.

• Demonstrated expertise in housing instability, related social determinants of health or mental health.

• Proven ability to collaborate effectively.

• Capable of working in both office and remote environments.
